What you will do:

- Conduct assessments of adults to identify needs and develop personalised care plans
- Collaborate effectively with hospital staff, healthcare professionals, and family members to facilitate smooth transitions from hospital to home
- Monitor and review care plans regularly to ensure they meet individual needs and adjust them as necessary
- Provide crisis intervention and safeguarding services to protect vulnerable adults
- Maintain accurate and up-to-date records and reports in accordance with legal requirements and organisational policies

Requirements of the Social Worker:

- Degree or equivalent in social work
- Current registration with Social Work England
- Significant practical experience in social services within the UK as a qualified Social Worker, demonstrating your expertise in adult social care and commitment to the profession
