We are excited to be advertising for a Band 8A Senior Clinical Site Practitioner to join our established Site Operations and Hospital @ Night team at the Princess Royal University Hospital and Orpington Hospital.

Site Operations is the centre of the hospital and responsible for patient flow with all capacity pathways and providing clinical and operational support out of hours.

We are looking for a dynamic, energetic and experienced individual who will successfully lead the 24/7 Clinical Site Management Services in and out of hours, and who will champion our patients, staff and services to provide the best care available from admission to safe discharge.

The job entails shift work of both days and night shifts as well as weekend working.

Main duties of the job

The post holder will be required to:

To take individual accountability and collective responsibility for the safe
running of the Clinical Site Operations & Hospital at Night (H@N)
service.

• Lead patient flow across the hospital 24/7

• Be accountable for providing senior nursing and management support, advice and clinical leadership to clinical areas within the trust, ensuring a safe clinical environment is provided for patients at all times, in and out of hours.

• Act as a visible presence as the Trusts’ senior operational and clinical manager, with responsibility in & out of hours for Trust standards, performance and infection control guidelines.

• Lead and develop the Clinical Site Team.

• To be accountable for the provision of high quality leadership to the Clinical Site Management & H@N teams and the co-ordination of the hospital handover, focusing on deteriorating patients, patients at risk, referrals and new admissions.

• To participate in and co-ordinate the Trust’s Emergency Response Teams to Fire, Security alerts, Cardiac Arrests and other clinical emergencies, and to be responsible for the co-ordination and first line management of major and serious untoward incidents, as required.

Detailed job description and main responsibilities

Clinical Practice
Clinical Site Management
• To co-ordinate a seamless service through the development of processes and communication with regards to bed management, emergency care performance and out of hours team.
• To ensure admissions are appropriately placed through assessment of medical, nursing and infection control needs.
• To work with and support the discharge coordinators and ward staff to ensure appropriate, safe and timely discharge for patients. Ensuring digital maturity with the use of EPIC at all times, ensuring live, accurate and updated information to support patient discharge and flow.
• To lead on the development of appropriate patient group directives to facilitate/enhance the discharge process.
• To work with key staff members to establish and provide detailed baseline patient assessment, education and support to patients and their families.
• To provide specialist nursing service for patients in conjunction with the out of hours team and H@N.
• To ensure and accept accountability that all patients have an accurate plan of care, which reflects the assessment undertaken and incorporates the issues and recommendations made.
• To be responsible for identifying and developing clinical protocols and strategies to enhance both the continuity and standard of care whilst ensuring equity of access to the service.


Senior Clinical Site Practitioner
• To be actively involved with the development of the Senior Clinical Site Practitioner role within the H@N Service.
• Develop an Operational Policy in conjunction with the new service and evolving service model in relation to assessment, treatment referral and care planning.
• To carry out ward and board rounds each night in the clinical areas, taking appropriate action on the findings of rounds and Early Warning Scores, reporting any adverse incidents.
• To provide senior nursing support, professional and clinical advice to all areas within the trust to provide a safe clinical environment.
• To lead the development of policies with regards to Treatment Pathways and referral systems.
• To undertake advanced patient assessment, advising on immediate care or referral for medical intervention ensuring the prompt and effective management of patients.
• To undertake a wide range of advanced clinical interventions which includes prescribing under Patient Group Directions, Non-Medical Prescribing and Advanced Life Support Skills
• To ensure all care is given in accordance with agreed protocols.
• To develop clinical policies and protocols relevant to the area in conjunction with senior nurses within the Care Groups, ensuring that practice is evidence based.
• To provide infection control advice, with the assistance of the on-call Microbiologist.
• To provide expert nursing advice and support to patients, their families and other healthcare professionals following diagnosis and through treatment.
• To support staff in the management and care of patients requiring level 2/3 type care and referral to critical care.
• To lead the “Handover” and allocation of tasks in relation to the teams skills.
• To respond to and provide senior nursing support in the event of a clinical emergency or as a member or the trust resuscitation team.
• To actively involve service users in providing feedback on their experience of the current service and suggestions for improvements.


Education and Research
• To provide information for clients, carers and health professionals about all aspects of care.
• To ensure appropriate information and educational literature is made available to help patients make informed choices as to their treatment plan and continuing management.
• To contribute to the development of nursing practice within the team.
• To ensure all new team members have a written formal induction to the team that includes formative and summative meetings where progress is measured against objectives.
• To actively participate in education events provided by the Trust.
• To attend meetings and conferences as appropriate ensuring that you arefully conversant with current issues within both the Trust and within the specialist services locally and nationally.
• To promote and facilitate evidence based practice and clinical audit within the Trust, raising the profile and enhancing the contribution of nursing to patient care.
• To participate in and assist with any appropriate trust approved research projects.
• To promote nursing research and evidence based practice relevant to the speciality.
• To work with and support clinical staff to identify areas where changes in practice are required. Also where opportunities exist for improving treatment and support of patients.
• To publish and present relevant clinical practice and service developments annually.


Management

• To lead and chair flow meetings 3 times a day
• To act as lead clinical site practitioner for the Trust out of hours.
• To lead and implement the Trusts Emergency Plans.
• To be accountable for the line management and support junior members of the H@N team, Clinical Site Practitioners and Technicians.
• To set and review individual, team and organisational objectives with direct line reports.
• To be accountable for the on-time delivery of individual, team and organisational actions, objectives and projects.
• To perform monthly 1:1 meetings, records to be kept and annual appraisals with direct line reports.
• To attend H@N team meetings and to deliver actions within the allocated time.
• To play an active and supportive role within the Trust bed management plans, to advise on the effective utilisation of the Trust bed pool.
• To be accountable for the initiation of escalation to Trust executive through correct channels, any issues regarding capacity that have not being resolved locally.
• To be accountable for and play an integral role in the operational and strategic development of the H@N service.
• To ensure that clinical and other untoward incidents, which include failureto meet related standards as set out by trust and local policy, are
investigated, reported upon and escalated appropriately to senior management.
• To be accountable for the implementation of any recommendations arising from the above investigations.
• To liaise effectively with all other disciplines within the Trust.
• To be accountable for and participate in the development of the Clinical Site Management and H@N team by maintaining accurate records for the audit process and by liaising with clinical areas and other service users to evaluate the effectiveness of the service.
• To hold individual accountability and collective responsibility for ensuring that the Clinical Site Management and H@N service operates effectively across the trust, maintaining financial balance, participating where necessary in providing cover for colleagues’ unexpected sickness/absenceat short notice.
• To be accountable and take a proactive part in management of change to broaden activities, standards and communication between; on site teams other Acute Trusts and Primary Care.
• To actively contribute to any relevant initiatives within the Trust and provide support to senior colleagues and Heads of Nursing.
• To contribute positively to the leadership of nursing within the Trust acting as an effective role model.
• To work with and support managers and clinical staff to identify areas where changes in practice are required.
• To be accountable for the introduction of measures to actively manage and reduce risk to patients. Ensuring the formal reporting and recording of adverse incidents is in line with Trust policy.
• To be accountable for Networking with the other H@N and other Clinical Site Manager teams to evaluate, develop and promote the specialistnursing role.
• To utilise and maintain information systems to aid audit and to provide regular actively analysis reports.
• To be accountable for the maintenance of contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity dataon time, as requested. To evaluate the service in terms of clinical effectiveness and value for money.
• Ensure compliance with the Trusts’ Equality and Diversity Policy, promoting equal opportunities at all times.


Personal Professional Development
• To be accountable in achieving a range of clinical competencies and educational attainment as required by the role of SCSP and within the
Trust.
• To be accountable for keeping up-to-date with current literature and research in the speciality.
• To be accountable for the maintenance of your own personal and professional development in accordance with revalidation requirements,
attending mandatory and statutory study sessions as required
• To undergo and actively participate in your own performance appraisals
• To keep up to date with NMC & Trust guidelines and protocols
• To adhere to the NMC Code of Professional conduct working within and accepting responsibility for maintaining agreed levels of competence

Person specification

Experience

Essential criteria
  • Experience of working autonomously at an advanced clinical level
  • Relevant experience in an acute setting
  • Extensive experience of line managing a group of staff
  • Experience in a Band 7 position and demonstrable competencies at that level

Desirable criteria
  • Experience chairing meetings or presenting

Computer literacy

Essential criteria
  • Relevant experience using Microsoft software, including Microsoft Teams
  • Able to complete and utilise excel spreadsheets and complete reports

Desirable criteria
  • Usage of sharepoint
  • Experience with EPIC

Education

Essential criteria
  • Higher degree in health related subject (or on recognised pathway)
  • Certificated competence in Enhanced Clinical Assessment
  • Evidence of post registration education at degree level

Desirable criteria
  • Teaching/Education qualification
  • Certificated competence in Non-Medical Prescribing
