What You Will Do:

Conduct thorough assessments to identify functional limitations and determine individual care needs

Deliver services according to the Care Act 2014 legislation ensuring comprehensive care act reviews are completed

Collaborate effectively within a multidisciplinary team to ensure the best outcomes for clients

Apply your strong knowledge of safeguarding principles and the Mental Capacity Act to safeguard vulnerable adults

Develop creative interventions tailored to the unique needs of each individual

Communicate clearly and effectively with clients, families, and colleagues to facilitate excellent care

Manage your caseload efficiently using strong organisational and time management skills
