To work with Adults with physical and Learning Disabilities as an Occupational Therapy Assistant Carrying out Assessment in the community.
JOB SUMMARY
Completing occupational therapy assessments, identifying and addressing occupational performance skills deficits, and planning OT intervention that will enable the client to meet needs in areas of self-maintenance, productivity and leisure.
Working with clients, relatives and carers to identify OT goals as part of an overall care plan, including organising and facilitating case conferences and family meetings.
Planning and implementing individual and/or group interventions, in collaboration with the client, using graded activity to achieve therapeutic goals.
Liaising closely with members of the MDT and other service providers as part of the care co-ordination process.
Communicating and working collaboratively with other agencies, such as: Care Agencies, Statutory Services, Voluntary Organisations, Social Services, Equipment.
