Mr J’s mental health had declined in which he was admitted to hospital. While he was being looked after we grew concerned about his personal care and lack of routine, and quickly realised that he needed social care support, to support his independence and help prepare him for a successful return to the community he loved.

He was assigned to Ms T, one of our experienced Mental Health Social Workers, who took a strengths-based, person-centred approach to his recovery. From their very first meetings, she built a strong rapport with Mr J and discovered he had a real passion for painting. She encouraged his creativity by providing art supplies and linking him with art therapy, helping him use his imagination and love of colour to manage his emotions and rediscover a genuine sense of purpose and fulfilment.

Working together, Ms T and Mr J developed a personalised plan for his discharge. This included re-establishing daily routines, building support networks, and engaging with a range of creative outlets. And so he could receive support and encouragement when he was back home, she also helped to connect him with a number of local art and mental health groups.

By the time he left hospital, Mr J felt much more positive, confident and hopeful about the future. His journey illustrates how our amazing Social Workers transform lives for the better, and focusing on a person’s creativity, strengths and connection to their community can make a world of difference to their mental health.

The Role:

As Placement Monitoring Officer, you too can make a powerful contribution. You’ll be joining Westminster Adult Social Services - Adult Care team. We’re a diverse, inclusive team with an incredible passion for our work. As part of our team, you’ll manage a caseload of adults with complex needs, regularly ensuring that their service plans are fit-for-purpose.

You will monitor the health and social care aspects of all Support Plans, ensuring that a good standard of care is being provided. Our service users are in residential placements, nursing homes and supported living facilities. We seek to respect their independence, dignity and privacy. To help us do this, you’ll establish relationships that promote trust and confidence. You’ll give each service user clear information about their rights, responsibilities and work with them to agree on a support plan with measurable outcomes that maximises their independence. You’ll carry out the following: holistic assessments, care checklists, and reviews into finances. Risk assessments will reflect a person’s capacity to make decisions and acknowledge that they have the right to take risks. If you’re ever concerned about abuse of a vulnerable adult (even if they are not allocated to you), you will undertake Safeguarding Adults Investigations and report this in line with policy.

As well as carrying out assessment, you’ll prescribe equipment, assistive technology and minor adaptions to maximize an individual’s independence.

Another aspect of your role will be liaising with Social Services to ensure that placements meet service specifications. And, when possible, you’ll work in partnership with family and carers, staying sensitive to how these relationships may be complex. About You:

To be suitable for the role, you’ll already be familiar with the legislation, statutory guidance and best practice that underpins social work and care for adults and health. You can take laws and official guidance and turn them into clear, workable policies and day‑to‑day practice.

At times, you’ve taken a leading role in multi-agency teams to ensure effective service delivery. You’re also experienced at managing budgets and competing priorities in a complex, controlled environment. You’ve constructed measures, metrics and performance indicators to shape and drive performance.

We’re looking for a highly experienced people manager, capable of working with staff and others to embed organisational values. Plus, you’ve worked diplomatically on a multi-disciplinary level.

You’re committed to Council policies in relation to Equal Opportunity, Customer Care and service delivery, and implement these in the way that you work.

You have a qualification in one of the following: Social Work, Occupational Therapy, or Nursing.
