We are serious about collaboration and co-production, and we work in better and more innovative ways because of this. For example, we have built strong connections with our Hackney Council colleagues in Housing, in Benefits and Homeless Prevention and in Children’s Services.

We currently have a number of positions in our Complex Care Service within Adult Social Care for team managers, senior practitioners and reviewing officers. This service covers case management and review of Hackney residents.

We are seeking someone who is passionate about supporting older and disabled adults who receive services in Hackney.

You will have excellent communication skills, including the ability to record reviews and to provide advice. You will also have an understanding of the resources and support available to enable older and disabled adults to maximise their independence
