Care Coordinator

Key Responsibilities

Your role as a Care Coordinator would include:

  • Ensure compliance with all office systems and procedures.
  • Ensure rota is allocated, maintained and assigned to appropriate staff
  • Updating of care plans and providing support to the Care Teams
  • Conduct Care staff supervisions and assist the other Care office staff with aspects of their role.
  • On call duties
  • Develop strong relationships with the customers, their families, and other healthcare professionals.
  • Maintain accurate and up-to-date records of care provided and any changes in customer’s needs.
  • Good communication and listening skills
  • problem solving ability and a drive to succeed.

Requirements

It’s important to us that all Caremark Care Coordinators have the following qualities:

  • Experience of domiciliary Care, Care Co-ordination, and a working knowledge of the care industry.
  • Previous experience with Access Care Planning and People Planner desired.
  • U.K. Driving Licence
  • Excellent communication skills and the ability to build rapport with staff, clients and their families.
  • NVQ level 2 or higher in Health & Social Care
