What You'll Be Doing:
Join our established Community Mental Health Team and support adults with complex mental health needs by:

* Undertaking comprehensive assessments in the community and clinic settings

* Developing and implementing personalised, recovery-focused interventions

* Supporting service users to develop skills for independent living and social inclusion

* Working closely with psychiatrists, social workers, CPNs and other professionals to deliver holistic care

* Contributing to care planning, risk assessments and discharge planning

* Maintaining accurate and timely clinical documentation

Requirements & Qualifications:
To be successful in this role, you'll need:

* A current and valid HCPC registration

* Previous experience in mental health (community setting desirable)

* Strong knowledge of mental health conditions, recovery models, and risk management

* Excellent communication and teamwork skills

* A commitment to delivering person-centred, evidence-based care
