Clinical Pharmacist (Prescriber)
- Provide clinical support across both GP practices, ensuring safe and effective prescribing.
- Conduct medication reviews, structured medication reviews (SMRs), and support long-term condition management.
- Work closely with GPs, nurses, and the wider MDT to optimise medicines use and improve patient outcomes.
- Assist with audits, QOF-related work, and clinical projects within the PCN.
- Support the development and delivery of repeat prescribing processes and medicines optimisation initiatives.
- Independent Prescriber (IP) qualification.
- CPP or equivalent experience.
- Proven experience working within a PCN or GP practice environment.
- Strong clinical judgement and the ability to work autonomously as well as part of a multidisciplinary team.
- Excellent communication and organisational skills.
