Nurse Practitioner - Practice Development
Job overview
- To work within the Care Group to provide a comprehensive patient-centred, nurse-led post-transplant follow-up service that includes patients who have undergone kidney transplants, simultaneous kidney/pancreas transplants and simultaneous liver/kidney transplants.
- To assist in the provision of high quality nursing care in a minimum of 3 clinics per week.
- To support the strategic development and management of the services provided to transplanted patients so that outcomes meet recognised national standards.
- To support the Band 7 CNS’s in the appropriate and effective utilisation of human and physical resources.
- To act up in the absence of your line manager with support.
- To support research, audit and measuring quality indicators within the specialty.
- To assist in the development of all staff within the multidisciplinary team, providing support, advice, formal and informal education
Main duties of the job
- To assist in the co-ordination of care of transplanted patients ensuring that patients are closely monitored and effectively supported to optimise their transplant function and quality of life.
- Monitor and make treatment interventions in order to manage symptoms associated with CKD and transplantation.
- To order, interpret and act on relevant tests and investigations in collaboration with the MDT.
- To manage the work-up for re-transplantation of suitable people with failing transplants, ensuring they have adequate knowledge and are both physically and mentally prepared for re-transplantation and activated pre-emptively whenever possible.
- To ensure patients understand the relevance of the clinic in their care and treatment.
- To support good communication occurs with external and internal agencies caring for the patient.
- To use expert communication skills (eg motivational interviewing) to support patients and their families to share decisions and plan their care as far as they wish and is clinically appropriate.
- To provide nursing cover for the Renal procedures/interventions team for annual leave and sickness.
- To discuss referral to GPs, community nurses and other medical specialities as necessary with appropriate clinician

Detailed job description and main responsibilities
- To ensure data collection is accurate and up to date.
- To ensure renal database reflects clinic work.
- To ensure the desired level of clinical performance is achieved within the relevant clinical setting.
- To ensure that the transplant clinic is involved in all Unit quality initiatives and represented on relevant working groups within the Unit.
- To ensure high standards are achieved in line with NICE, Renal Association and all other related clinical practice guidelines.
- To participate in the Trust’s patient satisfaction surveys and audits.
- To actively participate in the audit and evaluation of the clinics ensuring the service is modelled according to the needs of the client.
- To participate in the development and induction of clinical guidelines relevant to transplant services
Person specification
Competency and ability to perform the role
Essential criteria
- NMC level 1 or Level 15 registered Nurse
- Evidence of relevant experience of renal nursing
- Ability to work independently
- Evidence of continuing education and clinical and professional Development
Desirable criteria
- Knowledge of national and renal transplant policies and bodies including NHSBT, NICE and the renal registry. Is able to demonstrate understanding of their purpose and role
- Advanced clinical assessment skills or a willingness to develop these
- Advanced Clinical Assessment Qualification
- Degree or equivalent in Nursing related subject
