SEN Case Officer
Key Responsibilities: * Hold responsibility for a defined caseload of children and youth's.
* Lead the end-to-end EHC Needs Assessment process, ensuring compliance with statutory timescales (20 and 6 weeks).
* Work collaboratively with youth's, parents and carers ensuring a person centred approach . * Gather and quality-assure multi-agency advice (education, health, social care) to inform decisions.
* Draft, amend and finalise high-quality EHCPs with SMART, outcome-focused targets.
* Ensure outcomes incorporate Preparing for Adulthood principles from Year 9 onwards.
