\nThe successful candidate will be based within the Integrated Discharge Hub and be part of the Support at home Service working primarily with patients in St John’s Hospital to help facilitate timely discharge from hospital and support people to remain in the community, where they are medically well but need sort term support due to a change of circumstances
\nThe Integrated Discharge hub is an inter – agency multi – disciplinary team made up of hospital social workers, community care co-ordinators, reablement staff, discharge facilitators, AHP staff as well as representatives from different services.
\nThis is an exciting opportunity for the right person to work with a range of professionals and people to support the principles of Home First.
\nThe worker must have the ability to work as part of a team in a pro-active manner identifying opportunities for timely discharge whilst ensuring the patient has the appropriate level of support, which may incorporate the use of technology upon discharge from hospital.
\nTasks will be carried out in partnership with service users, family members, colleagues within Social Policy and other professionals and agencies as necessary to meet need. The worker will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments.
\nThe worker will be required to undertake essential training including in adult support and protection training.
\nThe worker must have the ability to travel throughout West Lothian.
\nBefore confirming appointment, you will be required to obtain PVG Scheme Membership through Disclosure Scotland.
\nThe skills and knowledge candidate must have: -

- SVQ 3 or HNC in Social Care or equivalent or relevant experience in a social or health care setting is essential. \n
- It is desirable for the candidate to have relevant assessment and care management experience in a social work practice team. \n
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
