The Bone Marrow Transplant (BMT) Coordinator Clinical Nurse Specialist (CNS) is a senior specialist nursing role providing expert coordination and care for patients undergoing haematopoietic stem cell transplantation (HCT). The postholder manages complex patient pathways from referral through transplant and follow-up, acting as a key worker for patients and families. Responsibilities include clinical assessment, donor coordination, MDT working, patient education, psychosocial support and service development. The role requires advanced haemato-oncology knowledge, strong leadership and communication skills, and collaboration with local, national and international teams to deliver safe, evidence-based, personalised transplant care

Main duties of the job

  • To work as an advanced practitioner and effectively deliver an expert nursing service within the field of hematopoietic stem cell transplantation (HCT) within a multidisciplinary team.
  • To manage a caseload of patients/clients effectively following HCT in accordance with agreed National and Local guidance, protocols and pathways of care
  • To ensure that patients and their families are fully informed and supported by providing clinical, emotional and psycho-social assessment and support in accordance with Personalised Care Standards
  • To assist in the development of and to deliver evidence based clinical nursing practice in line with current research, guidelines and standard operating procedures
  • To establish and maintain clear lines of communication within the specialty team, with the multidisciplinary team, referring hospitals, local Cancer Network and tertiary referral centres across the UK, providing advice, support and education where appropriate and to ensure cohesive multidisciplinary management of patients

Detailed job description and main responsibilities

Clinical responsibilities

  • Deliver an expert specialist nursing service for HSCT recipients and related donors in line with Trust Cancer CNS policies and national standards including JACIE, EBMT and HTA guidance.

  • Provide specialist clinical assessment, advice, psychosocial support and evidence-based care to patients, families and healthcare professionals.

  • Assess holistic patient needs and coordinate safe, effective and individualised care throughout the transplant pathway.

  • Contribute to operational policy development and service improvement within the multidisciplinary team.

Key worker responsibilities

  • Act as named key worker, ensuring accurate documentation and continuity of care.

  • Participate in inpatient and ambulatory care, supporting ward teams and promoting safe practice.

  • Support discharge planning, follow-up arrangements and patient decision-making.

  • Contribute to informed consent and provide accessible information regarding treatment and care.

  • Coordinate investigations, donor work-up, stem cell harvesting and transplant scheduling.

  • Assist with tissue typing and donor searches, including liaison with Anthony Nolan Trust.

  • Collate work-up results, identify concerns and take appropriate action.

  • Liaise with external departments and organisations, including radiotherapy services and community teams.

  • Participate in follow-up clinics and provide cross-cover for Haematology and BMT CNS teams when required.

  • Represent the service at Trust, regional, national and international meetings.

Personalised care

  • Deliver Personalised Care Key Performance Indicators in line with Trust guidance.

  • Support patients to self-manage through education, information sessions and personalised care planning.

MDT responsibilities

  • Attend and coordinate MDT meetings, ensuring all patients are represented.

  • Advocate for patients and contribute to integrated care pathways and MDT documentation.

  • Maintain effective referral pathways to the CNS service.

Patient experience and involvement

  • Promote patient involvement and gather feedback to support service improvement.

  • Participate in review of patient experience data and quality improvement initiatives.

Education, Research and Development

  • Support the development of nursing practice and contribute to appraisal and professional development.

  • Deliver education and training to staff, patients and carers.

  • Act as a mentor, assessor and clinical resource for colleagues.

  • Identify knowledge gaps and support staff education within the specialty.

  • Maintain awareness of current evidence, research and national developments.

  • Participate in audit, service evaluation and Trust-approved research activities.

  • Contribute to research and innovation within the specialty.

Leadership and management

  • Work collaboratively with multidisciplinary teams across primary, secondary and tertiary care.

  • Support service development, change management and quality improvement.

  • Promote awareness of the CNS role internally and externally.

  • Contribute positively to nursing leadership and act as a professional role model.

  • Ensure appropriate incident reporting, risk management and governance processes.

  • Maintain accurate records, activity data and audit information.

  • Provide specialist advice to colleagues and external agencies.

  • Ensure compliance with NICE guidance, service standards and Trust Equality and Diversity policies.

  • Support cost-effective and efficient use of resources.

Professional practice

  • Adhere to the NMC Code and maintain professional competence.

  • Achieve and maintain required clinical competencies.

  • Participate in clinical supervision, mentorship and reflective practice.

  • Maintain professional development and revalidation requirements.

  • Keep up to date with Trust, NMC and specialist guidelines.

  • Work towards higher academic or professional qualifications where appropriate.

Person specification

Education and qualifications

Essential criteria
  • NMC level 1 registered nurse
  • Educated to Degree level
  • Relevant post-registration qualification and evidence of professional development
  • An advanced practitioner either through formal qualification (Degree/ Masters) and/ or extensive clinical experience pertinent to the area
  • Advanced clinical assessment skills or willingness to develop these

Desirable criteria
  • Advanced communication skills and breaking bad news training
  • Level 2 psychological skills

Knowledge and Experience

Essential criteria
  • Extensive clinical knowledge and experience pertinent to Haemato-oncology
  • Experience of managing caseload of patients, working independently and as part of MDT
  • Experience of data collection for audit, analysing data and delivering presentations
  • To have demonstrable experience of service development
  • To have demonstrable experience of teaching others
  • Sound knowledge of Cancer Personalised Care and current NHS policy
  • Understanding of clinical governance and risk assessment

Skills and competencies

Essential criteria
  • Communication skills: ability to impart complex information to patients, relatives and carers, with different levels of understanding
  • Ability to build rapport with patients and MDT
  • Strong leadership skills
