Role Overview


ELFT are seeking an experienced Band 6 Physiotherapist to provide high-quality rehabilitation and community-based physiotherapy within the Referral and Assessment Team (RAT) in Newham.


The role focuses on delivering person-centred care to support patients in maintaining independence, reducing hospital admissions and supporting safe discharge planning within community and care settings.


Key Responsibilities


Provide physiotherapy assessment and treatment to patients referred to the Referral and Assessment Team


Deliver holistic, patient-centred care within patients' own homes and residential care settings


Carry out comprehensive case management including assessment, planning, implementation and evaluation of care


Support discharge planning alongside the Professional Lead and wider MDT


Hold responsibility for patient care within the RAT caseload


Support and contribute to the wider Newham multidisciplinary team


Liaise with community partners and services to support safe and effective patient care


Maintain high standards of clinical documentation and communication


Ensure high-quality service delivery using available resources


Essential Requirements


HCPC registered Physiotherapist


Band 6 experience (or strong Band 5 stepping up)


Community or intermediate care experience desirable


Experience with functional assessments and discharge planning


Ability to manage a community caseload independently


Strong communication and MDT working skills


Ability to travel across Newham


Desirable


Experience working within community rehabilitation or rapid response teams


Experience working in patients' homes or residential care settings


Knowledge of local health services and pathways
