Transfer of Care Lead
This post is a partnership role working across GSTT & colleagues from Southeast London (SEL) discharge services. They will be responsible for the development and delivery of discharge services. The post holder will work in line with agreed objectives reflecting the targets, quality standards, resource controls and constraints and will contribute to delivering the management strategy of the discharge service within the Trusts. The successful candidate will be a skilled communicator and be able to communicate highly complex and sensitive information at all levels across multiple Organisations. The post holder will be able foster strong relationships at senior levels with key strategic and delivery partners, across the NHS and wider health and care sector and expert a creating co-operation and collaborative in working across organisation boundaries. In line with these responsibilities, the post holder will oversee and ensure the development and coordination of strong delivery plans across the discharge/transfer of care Services. Main duties of the job
- Strategic and Operational Development: lead the strategic and operational direction of the discharge/transfer of care services across GSTT & SEL and effectively mainstream strategic priorities into commissioning and provider business across the health economy.
- People management : lead and develop high performing local delivery team, and line manage a team.
- Partnership working : effective relationship management and engagement with senior clinical and non-clinical stakeholder; developing strong and credible network relationships with partner Organisations within SEL.
- Design and lead delivery of significant transformational change regarding how we manage transfers of care/discharge, to meet trust strategic and operational objectives across the Trusts leading to measurable improvement in efficiency, safety and quality of care for patients.
- Lead effective implementation of the programme plan with appropriate monitoring arrangements in place.
- To implement a comprehensive programme dashboard effectively utilising existing data for the measurement of performance and modelling of outcomes, where possible working with IT colleagues in using electronic media to do so.

The post holder will:
- Lead on horizon scanning activities and identify local, national and international best practice to inform transfer of care/discharge services and pathways across Organisations and where appropriate ensure that it is adopted within the portfolio
- Extensive experience in synthesis and analysis of highly complex data from national and local sources and translating data to the Board
- Use complex data and skilled judgement in interactions with stakeholders to shape the design of the portfolio
- Promote innovation and research across transfer of care/discharge services, ensuring any research is translated into best practice
Education and Qualification
Essential
- Professional registration - Nursing, HCPC, Social Care
- Higher professional or management degree masters level or evidence of actively working towards this
- Evidence of relevant continuing professional development
- Post management qualification
Essential
- Recent experience of working at senior level in transfers of care and optimising complex patient pathways across organisational boundaries
- Experience of successfully managing significant organisational change
- Evidence of working across various healthcare/social services settings and representing the service both internally and externally
- Experience of advising and influencing senior managers and clinicians in relation to risk management and quality improvement
- Success in working collaboratively across professions and services.

- Demonstrable track record of improving care and services for patients.
- Proven ability to foster an organisational culture that is positive about change and committed to delivery of the strategic vision.
- Ability to respond to change efficiently and effectively, planning the implementation of fundamental changes arising from government policy which impact on the whole organisation.
- Strong analytical and problem solving skills.
- Substantial experience of working with a diverse range of stakeholders and a successful track record of influence and engagement.
- Ability to build and maintain networks across organisational and professional boundaries and to work strategically with other organisations.
