Community Social Services Occupational Therapist
JustOT are recruiting for an HCPC registered experienced Occupational Therapist to work with our well-respected client based in London.

Working Hours: Full-time, 35 hours per week, hybrid working Job Description:
To carry a caseload of varied range of complexity Adult's and Paediatrics
To take on care coordination responsibility for their caseload.
To undertake Mental Capacity Assessments in accordance with the Mental Capacity Act 2005
To ensure that the Care Act 2014 is implemented in all aspects of their role.
To work with service users, carers and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible.
To obtain informed consent for all care coordinated cases.
To ensure that their Senior Practitioner is kept informed of any significant events relating to the service.
To provide advice and consultancy to colleagues within the immediate team as well and to a wider range of stakeholders, which may include the NHS, the Independent sector and members of the public as appropriate.
To keep records of work undertaken and entering data on the database in accordance with Council Policy.
To participate in an urgent response system as directed by the Team Manager.
To promote the Personalisation agenda and support implementation of Self Directed Support
To facilitate development and validation of support plans and record unmet need.
To investigate and respond appropriately to complaints.
To attend multi- disciplinary, locality and other meetings as required.
To meet contractual agreement by attending regular supervision
To take responsibility for promoting and safeguarding the welfare of vulnerable adults in your care and those who you come into contact with
