Salary: £42,414 to £50,691 per annum

What you will do:
- Provide high-quality and effective social work services, responding to the needs of patients and their families
- Complete assessments and support plans according to the Care Act 2014, adopting a strengths-based approach
- Work collaboratively with the hospital team to facilitate smooth patient discharges
- Manage risks and support patients to enhance their independence
- Undertake strengths-based Care Act assessments for patients and their informal carers
- Participate in regular supervision and training sessions to continuously improve your skills
- Work in line with our Supervision Policy to ensure you receive guidance and support
- Engage in professional development as expected by Social Work England
