We have an exciting opportunity for a Release and Transfer Nurse Coordinator to join our Primary Care team at HMP Wormwood Scrubs , a category B Prison located in London.

In this role you will have the opportunity to provide the highest standards of healthcare to our patients, without having to compromise on efficiency and quality.

Hours
Monday to Friday: 07:30 - 15:30

Salary

This position is full time, you'll receive an annual salary up to £55,086 per annum - depending on experience.

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

What you'll be doing

As a Release and Transfer Nurse Coordinator your responsibilities will vary. You will
  • Follow and implement the Release and Transfer Pathway.
  • Oversee the management of assessments of health needs, planning, delivery and evaluation of care, including clinical risk assessments.
  • Manage face to face clinics with patients and being the experienced clinician for patients who may need a little more support for their release/ transfer.
  • Contribute to MDT discussions and where necessary working collaboratively with other healthcare professionals and prison colleagues for safer discharge planning.
  • Line manage other members of the Transfer & Release team and will work collaboratively with release teams in the prison along with other outside organisations.

Requirements
  • NMC Registered Adult Nurse
  • Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness
  • Experience of demonstrating high levels of clinical expertise
  • Line management experience
Although not essential ideal applicants would have experience in a hospital, community or prison environment.
