Release and Transfer Nurse Coordinator

In this role you will have the opportunity to provide the highest standards of healthcare to our patients, without having to compromise on efficiency and quality. HoursMonday to Friday: 07:30 - 15:30 Salary This position is full time, you'll receive an annual salary up to £55,086 per annum - depending on experience. How will we support you?

What you'll be doing As a Release and Transfer Nurse Coordinator your responsibilities will vary. You will
- Follow and implement the Release and Transfer Pathway.
- Oversee the management of assessments of health needs, planning, delivery and evaluation of care, including clinical risk assessments.
- Manage face to face clinics with patients and being the experienced clinician for patients who may need a little more support for their release/ transfer.
- Contribute to MDT discussions and where necessary working collaboratively with other healthcare professionals and prison colleagues for safer discharge planning.
- Line manage other members of the Transfer & Release team and will work collaboratively with release teams in the prison along with other outside organisations.

- NMC Registered Adult Nurse
- Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness
- Experience of demonstrating high levels of clinical expertise
