What you will do

* Conduct comprehensive assessments and create effective support plans for adults post-hospital discharge.
* Liaise with healthcare professionals, families, and carers to ensure seamless and safe discharge processes.
* Implement and review Reablement processes, supporting individuals to regain independence.
* Integrate the Discharge to Assess Pathway into your daily practices, promoting efficient transitions.
* Utilise your knowledge of the Care Act and other relevant legislation to uphold and advocate for the rights of service users.
* Leverage your expertise in using the Liquidlogic (LAS) systems for record keeping and case management.
* Provide exceptional support and guidance to adults, tailoring care to their specific needs.
