This is an exciting opportunity to engage with the community and make a positive impact on individual lives. Working as an Occupational Therapist within the Contact Team at Southwark, you'll be at the heart of helping people achieve the independence they desire.

What you will do:

Screening and Triage: Review and screen new referrals to determine urgency and appropriateness. Liaise with referrers, service users, carers, and other professionals to gather relevant information. Prioritise cases based on risk, need, and service criteria.

Initial Assessments: Conduct holistic, strengths-based assessments in line with the Care Act 2014. Identify occupational performance issues and develop person-centred intervention plans. Recommend and arrange appropriate equipment and adaptations to support independence.

Quality Assurance and Supervision: Provide guidance and oversight to assistant practitioners and unqualified social workers/OTs. Review and sign off assessments, support plans, and documentation to ensure quality and compliance. Offer mentoring, reflective supervision, and informal training as needed.

Multi-Disciplinary Working: Collaborate with social workers, health professionals, and community services to ensure coordinated care. Participate in case discussions, safeguarding meetings, and discharge planning.

Documentation and Reporting: Maintain accurate, timely, and professional records in line with legal and organisational standards. Contribute to service audits, performance monitoring, and continuous improvement initiatives.

Person Specification:

Essential: To qualify for this role, you must hold a Degree or Diploma in Occupational Therapy and have HCPC registration. Experience in adult social care or community-based OT services, along with strong assessment, communication, and decision-making skills, is required. The ability to supervise and support junior or unqualified staff is crucial. A driving licence is also necessary.

Desirable: Experience in front door or rapid response services, knowledge of relevant legislation such as the Care Act and Mental Capacity Act, and familiarity with electronic case management systems will be advantageous.
