What you will do:

Conduct independent assessments, treatments, and discharge planning for patients within the community rehab team.

Engage in equipment reasoning and ordering, ensuring the right tools are available for effective patient care.

Utilize recognisable outcome measures to track patient progress and outcomes.

Host family meetings to discuss treatment plans, adjustments, and progress.

Collaborate with a multi-disciplinary team, sharing insights and strategies for patient care.

Drive to various community settings, so a valid driving licence is essential.

Requirements: To be successful in this role, you must hold HCPC registration and ideally have experience with the NHS and post-qualification as an Occupational Therapist within community and rehabilitation settings.
