About the people and their home
Old street is a residential home that supports adults with learning disabilities and epilepsy where up to five people can live.
At Old Street, an individual’s needs and goals are identified through ‘person centred planning.’ The people that we support are at heart of everything we do. We support them in making every day meaningful through excellent care and support.
We support integration into the community through social opportunities.
Person centred support plans are created through keyworker sessions where people are supported to develop their goals and dreams.
People are supported to choose their keyworker and to have monthly meetings with that person. The meetings are done in their preferred communication method.
We support them to explore new activities and decide what they would like to do with their time. We support people to have a personalised activity plan.
We support people to attend an annual health check and support to a GP appointment when they feel unwell.
We support people to store and take their medication. We support people with medication reviews to make sure they are not taking medications that they may not need.
We support people to make their own choices and decisions. The people we support choose when they want to go to bed and when they get up in the morning.
We support people and their families to have regular contact and enjoy quality time together.
We do not have house rules at Old Street but ask that people who live at Old Street do not harm others or their belongings.
The Home Manager is Nerea. She has worked in the care industry for over 10 years.
The staff are trained internally and receive updates to this training regularly to ensure the needs of the people we support are met.
Staff are supported to complete diplomas and health and social care to give them greater knowledge and skills.
People we support have access to a company vehicle for travel to local facilities and activities.
About you
Although it would be beneficial to have some experience of healthcare work, it isn’t absolutely necessary as we provide full training. As long as you’re passionate, hard-working and empathetic, you’ll fit right in.
