They’d always lived together and were terrified they’d be split up. Our social worker did a great job persuading Mrs D to get hospital treatment and a temporary stay in a care home. In an epic effort throughout COVID, she gained the son’s trust and sorted all the repairs, carers and nurses needed so that Mrs D could come home to live with her son. We also got Mr D regular respite and a personal budget awarded in recognition of him as her carer.

The Role:
As a Social Worker with our Hospital Discharge Team you can make your own powerful contribution to Westminster’s success. Based in St Mary’s Hospital and operating across the Borough of Hammersmith and Fulham, the Royal Borough of Kensington and Chelsea and Westminster City Council, you’ll work closely with health colleagues within an acute setting, and be an active member of a ward multi-disciplinary team. Undertaking assessments of need and developing support plans that help patients with their discharge from hospital, you’ll also undertake safeguarding investigations and Mental Capacity Act assessments, and work with carers and other professionals to minimise and manage risk while helping each patient to maintain their chosen lifestyle.
Offering advice and information to your colleagues and other stakeholders on the role of the social worker and the service we provide, you’ll join our urgent response and duty system and participate in developments to our service. Committed to ensuring that your professional registration and DBS are kept up to date, we’ll also expect you to be responsible for your own CPD, and participate in appraisals, audits, evaluations, reflective practice and team meetings.
It’s important that you work in line with all our policies and procedures including health and safety, data and case recording, supervision, data protection and information governance, and you should be ready to inform your manager of any significant events relating to the service. Dedicated to undertaking best interest and practice teacher training as part of your development, you’ll also take part in Trusted Assessor training – this will enable you to prescribe simple equipment and assistive technology that helps to maximise your customers’ independence. With this in mind, you’ll adopt an inter-disciplinary approach to your work and collaborate with all our partners in order to achieve best outcomes.
You should be aware that this role involves working five days a week including Saturdays, Sundays and Bank Holidays. You’ll work as part of a team for three days a week, and although you’ll be the only Social Worker on site over weekend and holiday periods, a Duty Manager will be available remotely to provide support.

About You:
To be a success in this vital role you should have a CQSW/Diploma in Social Work, a Degree in Social Work or its equivalent, be registered as a member of Social Work England, and have some post-qualifying experience in a relevant Local Authority or Health setting. It’s important that you have well developed knowledge of relevant legislation, a good understanding of the principles of personalisation, independence and choice, and experience of working with older or physically disabled people in a Social Services Duty System.
You’ll have worked in both a multi-disciplinary way and an acute setting, and in addition to possessing experience of assessment work, you’ll enjoy working in a collaborative team environment. Capable of carrying out accurate assessments and promoting self-directed care, your superb written and verbal communication skills will see you interacting well with everyone you work with, and producing high quality reports for carers and other professionals. With the ability to work independently, use your initiative, prioritise workloads and meet deadlines, you should also have confident computer skills, with a particular focus on Microsoft’s suite of software.
Able to negotiate with other agencies on behalf of the Commissioning Authority and service users, you’ll be a pro-active worker with the skills to pre-empt, prevent and manage any issues that arise. It goes without saying that you’ll have good case recording and assessment skills, an understanding of budgets and financial controls, and the ability to complete robust risk assessments. In addition, the ability to communicate professionally in a language other than English would be desirable.
