Main Purpose of the Job/Key Objectives:

• To undertake a key role in the Children and Adults with Disabilities Service. The team is responsible for providing direct support to children, young people, adults and their families and carers. They will also work very closely with early years settings, schools, colleges and other settings.

• The primary purpose of this role is to work with children and young people with special educational needs and disabilities and their families and carers and to contribute to the development of a care service that, based on the “Asset Model” approaches, delivers improved outcomes for these service users.

• To facilitate person centred planning meetings for children and young people with special education needs and disabilities (across the age range).

• To support children and young people and their families and carers to negotiate the newly established pathways for these service users, supporting their knowledge and understanding of the process.

• To use the “Asset Model” approach as a basis for their support for service users to empower families to develop choice and opportunities.

• To work closely with Health colleagues to ensure there is a seamless provision of support from birth.

◼ To demonstrate a flexible approach in the delivery of work within the service area. Consequently, the post holder may be required to perform duties not specifically identified in the job profile but which are in line with the general responsibilities of the post.

◼ Potential requirement to work at any Council health provider or education site.

◼ Weekend and evening working is a regular feature of this role.

NB: Where an employee drives their own vehicle on Council duties, the car insurance policy must cover business use.
