The post is with Adult Social Care- East Locality Team Client group – Older People, Adults with disabilities and vulnerable adults Type of work: Care Act Assessments, Support Planning and Reviews. Other areas include CHC Assessments, Asylum/NRPF, Transition and Safeguarding Experienced qualified Social Worker required; * Undertaking comprehensive social care assessments and or reports, with detailed support plans, as appropriate. * Knowledge and understanding of Direct Payments using the strength-based approach. * Social Worker should have sound knowledge around the continuing health care process and procedure and the legislation that underpins this, including the Decision Support Tools. * Social worker will also need to communicate closely and sensitively with family members of service users. Work arrangements: Flexible arrangements to include Work from Home, and office as required. Completing home visits where necessary and face to face meeting
