Community Case Manager RMN / RNLD

As a Community Case Manager RMN / RNLD, you will:
- Manage and oversee complex care packages for both paediatric and adult clients in the community.
- Mentor and train nurses and healthcare assistants to enhance their skills and service delivery.
- Develop and maintain strong relationships with commissioners and Integrated Care Boards (ICBs).
- Serve as a reliable point of contact for clients, families, and staff.

The ideal Community Case Manager RMN / RNLD will have:
- Active NMC registration as a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD).
- At least 2 years of post-qualification experience.
- A background in community mental health or Tier 2 CAMHS.
- A full UK driving licence and access to a vehicle.
