What you will do

* Conduct high-quality assessments and reviews, including annual reviews and progression reviews, to identify individual needs and aspirations.
* Develop and implement strengths-based, outcome-focused care and support plans that promote independence, resilience, and community inclusion.
* Coordinate timely interventions and support planning at key transition points.
* Work collaboratively with health professionals, carers, families, advocates, and partner agencies to deliver coordinated support.
* Support adults with learning disabilities, autism, and complex needs to achieve positive outcomes and reduce long-term dependency.
* Promote choice, control, independence, dignity, and wellbeing in all aspects of practice.
* Undertake Care Act assessments and reviews in accordance with relevant legislation and local procedures.
* Manage a varied caseload while maintaining accurate and timely case records.
* Participate in safeguarding processes and contribute to risk assessment and risk management activities.
* Engage in regular supervision, reflective practice, and continuous professional development.
* Contribute to service improvements and the development of innovative approaches to supporting adults with learning disabilities.
