Complex Care Nurse – Domiciliary Care
Role Overview
A Complex Care Nurse in domiciliary care provides specialist clinical nursing support to adults with long-term, complex, or life-limiting conditions in their own homes. The role focuses on delivering safe, person-centred care while promoting independence, dignity, and quality of life.
Key Responsibilities
Clinical Care
Deliver high-quality nursing care to clients with complex needs (e.g., neurological conditions, spinal injuries, learning disabilities, respiratory conditions)
Manage and monitor long-term conditions and complex care packages
Administer medications, including injections, IV therapies, enteral feeding, and oxygen therapy
Perform advanced clinical procedures such as:
Tracheostomy care and suctioning
Ventilation (invasive and non-invasive)
PEG/NG feeding
Catheter and stoma care
Wound and pressure ulcer management
Recognise and respond to clinical deterioration and emergencies
Care Planning & Assessment
Conduct holistic assessments and develop personalised care plans
Review and update risk assessments and care plans regularly
Ensure care delivery aligns with evidence-based practice and clinical guidelines
Support hospital discharge planning and admission avoidance
Leadership & Support
Train, supervise, and assess the competency of healthcare assistants and support workers
Provide clinical leadership within the client’s home environment
Offer education and emotional support to clients and families
Act as a clinical role model, promoting best practice
Multidisciplinary Working
Work collaboratively with GPs, consultants, therapists, social workers, and community teams
Attend MDT meetings and case reviews
Act as an advocate for the client, ensuring their voice and preferences are respected
Governance & Safety
Maintain accurate, timely clinical documentation
Ensure compliance with CQC/NMC standards, safeguarding policies, and infection control
Participate in audits, incident reporting, and quality improvement initiatives
Maintain professional registration and ongoing CPD
Skills & Competencies
Strong clinical assessment and decision-making skills
Excellent communication and interpersonal skills
Ability to work autonomously in a community setting
Compassionate, patient-centred approach
Effective time management and organisational skills
Confidence in managing complex and unpredictable situations
Qualifications & Experience
Registered Nurse (Adult / Learning Disabilities) – NMC registered
Experience in complex care, community nursing, ICU, or specialist settings
Up-to-date clinical competencies relevant to complex care
Full UK driving licence (with own Car)
Working Environment
Client’s homes and community settings
Flexible working patterns (days, nights, weekends, depending on care package)
Lone working with on-call and clinical support
About The Role
Assess clients with complex health needs in their own homes
Develop, implement, and regularly review individualised care plans
Deliver advanced clinical care (e.g. tracheostomy care, ventilation, PEG feeding, catheter and stoma care)
Safely administer and manage medications and therapies
Monitor clinical condition and respond to deterioration or emergencies
Maintain accurate, timely clinical documentation and reports
Train, supervise, and assess competencies of healthcare assistants
Support and educate clients, families, and carers
Liaise with GPs, consultants, and multidisciplinary teams
Identify and manage risks within the home environment
Ensure compliance with NMC Code, CQC standards, safeguarding, and infection control policies
Participate in audits, reviews, and continuous professional development
